Each and every patient must complete a Medical History Questionnaire when joining our practice. You will be asked to complete a new form annually to make sure that any changes to your medical details are noted in your dental patient file and that we are kept up to date with anything that may affect how we provide your care.
To save time, you can download this form, complete it at home and bring it along to your appointment. Medical History Questionnaire

This is a combination of questions about your previous dental history, any anxieties you may have and it also includes a small section where we ask you to tell us what you would like from us in terms of your ongoing dental care.
